Nicotinamide Riboside Protects the Heart from Blood Flow Injury: Preclinical Findings


Synopsis

Myocardial ischemia-reperfusion (I/R) injury damages the heart when blood flow returns to previously blocked tissue, causing oxidative stress, inflammation, and cell death. This study tested nicotinamide riboside (NR), a precursor of NAD+, for heart protection. Giving NR to mice before I/R injury improved heart function, reduced heart chamber enlargement, and restored key measures of heart performance. NR also protected heart cells from apoptosis both in mice and in lab-grown cells, lowering harmful proteins and restoring healthy ones. Mechanistically, NR worked by activating the SIRT3/mtROS/JNK pathway, reducing mitochondrial stress and blocking harmful signaling. In mice lacking SIRT3, NR no longer provided protection, showing its effects depend on this pathway. Overall, NR significantly protects the heart from I/R injury and could be a promising therapy for ischemic heart disease.
